Sciatica That Won’t Go Away: Why Painkillers Often Don’t Work

Painkillers rarely resolve persistent sciatica because they mask the pain signal without addressing the structural problem causing it. When the sciatic nerve is compressed by a herniated disc, a misaligned vertebra, or spinal stenosis, no amount of medication corrects that underlying pressure.

Why Painkillers Don’t Work for Sciatica

Sciatica is not a muscle ache or a soft tissue inflammation you can simply medicate away. It is a nerve compression problem.

The sciatic nerve, the longest nerve in your body, originates in the lower back and travels through the hips, buttocks, and down each leg. When something physically presses on that nerve, whether a bulging disc, a herniated disc, or a spinal misalignment, the pain, numbness, and tingling you feel are symptoms of that mechanical pressure.

Pain medications, including ibuprofen, acetaminophen, muscle relaxants, and even prescription opioids, work by blocking or blunting pain signals in the nervous system.

They do not decompress a disc.

They do not restore spinal alignment.

They do not reduce actual nerve pressure.

A 2012 systematic review published in BMJ found that existing evidence for the efficacy of drugs commonly prescribed for sciatica, including NSAIDs, corticosteroids, and opioids, is of low quality and does not clearly show favorable effects even compared to placebo.

UK medical guidelines now actively discourage the use of gabapentinoids, corticosteroids, benzodiazepines, and opioids for sciatica. A 2017 trial published in the New England Journal of Medicine found that pregabalin, a commonly prescribed nerve pain drug, did not significantly reduce leg pain from sciatica compared to placebo, while causing higher rates of dizziness, confusion, and sedation.

The medication carries real risk, with no reliable structural benefit.

What Is Actually Causing Your Persistent Sciatica

Persistent sciatica pain that has lasted weeks, months, or years, almost always has a clear structural cause that has not been fully corrected.

The most common underlying causes we see in our sciatica patients include:

  • Herniated or bulging disc: The soft inner gel of a disc pushes outward and presses directly onto the sciatic nerve root in the lumbar spine, producing sharp, radiating pain from the lower back into the leg.
  • Spinal stenosis: A narrowing of the spinal canal that places ongoing pressure on nerve roots, often causing pain that worsens with standing or walking.
  • Degenerative disc disease: Disc height loss over time reduces the space through which nerve roots exit the spine, creating chronic irritation.
  • Atlas misalignment: When the atlas, the top vertebra in the spine, shifts out of its normal position, the entire spine compensates by tilting and twisting. This compensation creates abnormal stress on discs, joints, and nerves all the way down into the lower back, including the sciatic nerve. This is one reason sciatica can be so difficult to treat when you only focus on the lower back.
  • Piriformis irritation: The piriformis muscle in the buttock can tighten and irritate the sciatic nerve where it passes beneath or through it, mimicking disc-related sciatica.
  • Chronic inflammation: Ongoing disc pressure and spinal misalignment restrict normal blood and nutrient flow to damaged areas, slowing the body’s natural healing process and keeping the nerve in a state of sustained irritation.

Recognizing Persistent Sciatica Symptoms

Sciatica does not always feel the same from person to person. The nerve covers a large area of the body, and where and how it is compressed affects the pattern of symptoms.

Common signs that your sciatica has not resolved include:

  • Sharp, shooting, or electric pain that travels from the lower back through the buttock and down one leg
  • Burning or aching pain along the back of the thigh or calf
  • Numbness or tingling in the leg, foot, or toes
  • Muscle weakness in the affected leg
  • Pain that worsens when sitting for prolonged periods
  • Symptoms that fluctuate but never fully disappear, even with rest or medication
